Created by Doctors of Physical Therapy, Columbia Care Home combines high-acuity clinical oversight with the calm environment of an intimate 8-resident residential home, a 1:3 caregiver ratio, and 24/7 overnight awake staff.
Our care model prioritizes individualized attention over institutional scale. Instead of large corridors and rotating staff, residents receive consistent supervision, mobility support, and personal care within a calm residential environment.

"As a therapist who goes into many assisted living facilities throughout the Howard County region, I wanted to take a moment to recognize the exemplary care provided at Columbia Care Home. The residents always appear well kept, caregivers genuinely care about the residents and are actively engaged in the residents’ care. I have found the owners to be approachable, receptive, and readily available for questions."

"The care my father, who has been diagnosed with late stage Parkinson’s, is receiving at Columbia Care Home is phenomenal. Bhargav, the owner, along with his wife and aides have been a blessing to our family. The attention to detail in both the patient care and the facility setup is like no other. A holistic approach to care is provided by team of clinical providers, various therapies and medication management."

"The caring and supportive team at Columbia Care Home is beyond amazing. The founding principles of the home are evident in their approach and they are also very calming and helpful to us as a family. The attention to detail and warmth in the updated environment are a comfort to the family. The team responds promptly to calls, texts, emails and help navigate changes."

"When families tour large facilities, they often see beautiful common spaces. What is harder to see is how individualized care becomes when dozens of residents require attention at the same time."
Columbia Care Home was founded by Bhargav Patel and Sheetal Khadsare, Doctors of Physical Therapy who spent years treating seniors in massive corporate facilities. They saw firsthand how difficult it was for large facilities to consistently provide individualized mobility supervision and behavioral support for residents with complex needs.
They built this 8-bed residential model to provide a radical alternative: a 1:3 caregiver ratio where clinical oversight is proactive, not reactive. Our care model focuses on proactive mobility routines, structured daily rhythms, and 24/7 overnight awake staff to support both physical and cognitive well-being.
Many residential homes are designed for basic assistance. Because our home is led by Doctors of Physical Therapy, we are equipped to support seniors experiencing mobility decline, fall risk, and increasing care needs.
From 2-person transfers to severe fall-risk management, our staff is trained directly by Doctors of Physical Therapy to handle significant physical decline securely.
View rehab protocolsMany facilities discharge residents when care becomes "too difficult." Our Level 3 license, 1:3 ratio, and overnight awake staff ensure we can support your loved one through late-stage decline.
Explore residential careWe use biomechanical daytime routines and environmental controls to reduce late-day anxiety, minimizing the need for heavy chemical sedatives.
