The Care Points Trap: What the Big Facilities Don't Tell You (And How to Avoid Them in Columbia)

"We were told one rate. By month three, we were paying nearly double."

We hear this all the time from families who come to us after a bad experience somewhere else.

Here’s what we’ve seen happen in most large facilities.

We don’t use a point system. The price doesn’t change based on tasks.

Unpredictable billing models can turn an affordable base rate into a significant financial burden.

1. The Base Rate Illusion

Most large facilities in Howard County advertise a low base rate to get you in the door. What they often don't emphasize is that the base rate only covers "room and board"—the physical space and your meals.

What happens in practice: the moment your loved one needs help, the price goes up. Most families only see it on the first bill.

Our initial assessment was close to 25% higher with hidden fees—even though our family member needs little to no personal assistance. If there is a need for personal assistance, the cost goes up by multiples of $100 per day.

2. The Anatomy of a "Care Point"

Many facilities assign a cost to each task. If your parent needs more help this month, your "points" go up, and so does your bill.

This is how it usually shows up on a bill:

Typical "A La Carte" Point Charges

Service Item Typical Monthly Cost
Medication Administration $100 - $145 extra
Escort to Dining Room $300 - $500 extra
Morning Dressing Assistance $400 - $600 extra
Incontinence Management $800+ extra

Charging separately for basic care starts to feel wrong, especially when it’s something your parent needs every day.

They are charging her $145 in "care points" each and every time they have to come to her room to administer drops! You already get a huge basic rent bill...

3. The Reassessment Shock

After a hospital stay or change in condition, many facilities reassess the level of care. This often leads to a higher monthly cost. Sometimes the price goes up even when the day-to-day care looks the same.

August was about $1,000 more expensive. I saw then that her level of care had been changed to a Level Two... They were just using her hospital stay as an excuse to squeeze more money out of us.

The Emotional Toll of Unpredictability

When the bill keeps changing, trust erodes. Families start feeling stuck. Moving again is hard, and facilities know that. This is one of the reasons families start looking at smaller homes.

We do this differently

  • No care points: The price doesn’t change based on tasks.
  • No changing levels: We do not use levels that increase the price. We provide Level 3 high-acuity care as our standard.
  • Transparent rate: Our monthly rate is usually between $7,000 and $10,000. It includes care, meals, medication management, and staffing. See exactly what's included in our pricing.

Questions to Ask During Your Next Tour

If you're touring other places in Columbia or Ellicott City, ask these:

  1. "Can I see a detailed point-to-dollar conversion chart?"
  2. "If my parent has a hospital stay, how does that affect their care tier immediately upon return?"
  3. "Is there any task that is NOT included in the base rate?"

FAQ: Assisted Living Costs in Maryland

What are "Care Points" in assisted living?
Care points mean you are charged for individual tasks like dressing, medication help, or mobility. Your total points determine your monthly care fee on top of your base rent.

What is the average cost of assisted living in Maryland?
While broad averages suggest $5,000–$7,000, families in the Columbia and Howard County area often find that high-quality, high-acuity care ends up costing $8,000–$11,000 once all add-ons are included. You can read more in our full breakdown of Maryland care costs.

How do I avoid hidden fees in assisted living?
The best way is to look for residential care models (like 8-bed homes) that offer all-inclusive pricing. Always ask for a written list of every service that incurs an extra charge.
